Typical number of sessions

Most clients require two sessions for 3D areola tattooing:

  • Session 1: consultation, colour selection, sizing, positioning, base pigment and 3D shading.

  • Session 2: usually 3-4 weeks later, after the skin has healed, to refine symmetry, restore pigment loss and add detail.

Some clients achieve a satisfactory result in one session, while others need three sessions or more, particularly where there is significant scar tissue, radiotherapy damage, uneven pigment retention, complex reconstruction or a demanding colour match.

Uneven pigment retention after the first 3D areola tattoo session is usually caused by a combination of normal healing differences, tissue condition and aftercare. It does not necessarily indicate poor technique or a problem with the pigment.

Common causes

  • Uneven skin quality: Scar tissue, radiotherapy-affected tissue, grafted skin and reconstructed breasts may absorb pigment differently across the treatment area.

  • Different healing responses: Swelling, lymph, scabbing and flaking are not always uniform. A thicker crust can remove more pigment as it separates.

  • Oily, thin or fragile skin: Sebum and fragile tissue can reduce retention, while mature or fibrotic areas may hold pigment unpredictably. Skin can also behave differently from one breast to the other.

  • Aftercare differences: Excess washing, rubbing, picking flakes, applying unsuitable products, heavy sweating, swimming, saunas or early sun exposure can remove or fade pigment unevenly.

  • Bleeding or fluid during treatment: Excess bleeding or lymphatic fluid can dilute and displace pigment before it settles.

  • Existing scar colour and texture: Scar tissue may have reduced circulation and altered collagen structure, making colour uptake less predictable.

  • Individual biology: Immune activity, hormones, medications, general health and healing capacity affect how quickly pigment particles are broken down or retained.

    Is the tattoo actually 3D?
    No, it is not physically raised. The 3D effect is created through careful colour layering, shading, and placement to make the nipple appear realistic.

    Does it hurt?
    Most people find the procedure very manageable, and some feel little to no pain, especially after surgery when sensation may be reduced. Comfort varies depending on the area being treated and the individual’s sensitivity. Anesthetic cream can be advised on consultation.

    How long should I wait after surgery?
    You usually need to wait until healing is complete and your surgeon or treating team clears you for tattooing. Many sources recommend waiting at least 12 months after final reconstruction, and some clinics prefer 12 to 18 months or longer depending on healing.

    How long does the tattoo last?
    The results are long-lasting, but the pigment may soften over time and may need a future refresh. Longevity depends on skin type, healing, sun exposure, and aftercare.

    Can it be done over scars or implants?
    Yes, areola tattooing can often be performed over reconstructed breasts, scars, or implants, although scar tissue may hold pigment differently. A consultation is important to assess suitability and expected results.

    What is the healing process like?
    Healing is usually straightforward, with mild redness, tenderness, or light flaking for a short period. Aftercare commonly includes keeping the area clean, avoiding friction, and following all post-treatment instructions carefully.

    Can it be matched to the other side?
    Yes, if one side is still natural, the tattoo can be colour-matched and shaped to closely complement the existing areola. A skilled practitioner will assess symmetry, tone, and size during consultation.
